Key Responsibilities\n

• Create and maintain a manageable caseload of clients, fostering positive relationships built on trust, professionalism, and empathetic understanding.

• Conduct regular sessions with clients to review treatment progress, schedule follow-up appointments, or initiate discharge planning when appropriate.

• Develop, implement, and monitor person-centered treatment plans tailored to each client’s unique goals and circumstances.

• Perform thorough intake assessments to gather comprehensive information on clients’ mental health histories and current needs.

• Document client interactions and treatment plans accurately, ensuring all progress notes are up-to-date and reflect client advancements and challenges.

• Participate in individual supervision to enhance clinical skills and provide a platform for discussing complex cases.

• Engage in collaborative efforts with other healthcare professionals to coordinate care and share insights on client progress.

Patient Population / Client Focus\n

This position focuses primarily on youth clients who face various mental health challenges including anxiety, depression, trauma, and behavioral issues. Qualifications\n

• A Master's Degree in mental health counseling or a closely related field.

• Current and valid licensure as a Licensed Clinical Professional Counselor (LCPC) in the state of Illinois.
