Key Responsibilities\n

• Effectively communicate with medical physicians to update them on patient progress, addressing any needs, concerns, and potential issues that arise during treatment.

• Maintain accurate, detailed, and timely clinical documentation and progress notes, following the organization's standardized policies.

• Collaborate closely with a multidisciplinary healthcare team in developing treatment plans, utilizing psychosocial assessments to inform appropriate therapeutic interventions.

• Assist clients with their discharge planning processes and conduct care coordination with community services to facilitate access to continued support post-treatment.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.

• Possession of an active state license as an LCSW, LMFT, LPC, or a comparable credential.

• Minimum of 3 years of experience working in an outpatient mental health setting.

• Proficiency in computer applications including word processing, email, spreadsheets, and electronic medical record systems.

• Strong organizational skills, with the ability to work independently and manage multiple client caseloads efficiently.

While the minimum requirement stands at 3 years in outpatient services, further experience in collaborative treatment planning, crisis intervention, and trauma-informed care will be considered a valuable asset.
