Registered Nurse – Home Health – Wilmette, Illinois Graham Healthcare Group is seeking a registered nurse (RN) to join our extraordinary care team at Residential Home Health and Hospice in Wilmette, Illinois. The RN will lead and collaborate in managing and delivering patient care plans within a team‑based environment. Job Description & Requirements Specialty: Home Health Discipline: RN Duration: Ongoing Hours: 40 per week, 8‑hour shift Employment Type: Staff The RN will support patients in Evanston, Skokie and surrounding areas, leading the delivery of individualized care plans and ensuring high‑quality, timely charting. Key responsibilities include: Collaborate with interdisciplinary teams to develop and manage patient care plans. Provide direct home health nursing services, including assessment, education, medication management, and wound care. Complete accurate and timely documentation in the electronic medical record. Maintain a high standard of patient safety and care quality. Qualifications RN license in the state of practice. Graduate from an approved practical nursing school. At least one year of RN experience with clinical competence for quality patient care. Current driver’s license and willingness to spend approximately 20% of your day driving to patient locations. Commitment to consistently meet critical charting deadlines. Self‑management skills for time and schedule. Experience with tablets, mobile phones and EMR software. Successful completion of a background check, including a drug screen and evaluation of medical and recreational marijuana policies.
