This position will primarily be a remote role, allowing you the flexibility of a work-from-home arrangement. Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the unique needs of each patient, tailoring care plans accordingly.

• Facilitate open communication among supervisors, family members, guardians, and other relevant parties to provide updates on client progress.

• Develop and share a flexible schedule of appointments, home visits, and observations to ensure proper follow-up on patient care.
\n
• Coordinate home visits and conduct thorough observations, chart reviews, and staff interviews to support effective treatment delivery.
\n
• Monitor the implementation of program plans to ensure effectiveness and high standards of care are maintained.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing valuable insights to enhance patient outcomes.
\n
• Complete monthly and quarterly progress notes that reflect ongoing patient development and necessary adjustments to treatment plans.
\n
• Organize training sessions for staff members to certify their competency in executing program plans and handling documentation efficiently.
\n
• Provide follow-up interviews and home visits to ensure ongoing patient support and adjustment of care as needed.
\n
• Maintain precise records to ensure immediate access to important information for all stakeholders.
\n
• Act as a liaison with the broader public and mental health community, assisting in the achievement of developmental goals.

• Assist in formulating training materials aimed at enhancing organizational programs and ensuring best practices are upheld.

Patient Population / Client Focus\n

This role focuses on a broad spectrum of clients, including children, adolescents, and adults facing mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is essential.

• Possession of an active and unrestricted LPC license in Arizona is a prerequisite.

Compensation and Benefits\n
