Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients and their families to identify emotional and social strengths, as well as challenges related to specific diagnoses and life situations.

• Develop individualized treatment plans that incorporate a variety of therapeutic interventions, including crisis intervention and both individual and group therapies.

• Collaborate with medical professionals and engage patients and families in the planning and implementation of care strategies.

• Facilitate discharge planning in conjunction with healthcare professionals, addressing any post-hospitalization needs and ensuring seamless transitions.

• Provide education and counseling to patients and families regarding coping strategies and adaptation to illness.

• Engage in on-call responsibilities to respond to after-hour inquiries and facilitate urgent patient needs.

• Utilize the standards of care, including evaluating patients for mental health issues such as depression and anxiety, and providing short-term counseling as necessary.

• Serve as a resource for other health care staff, contributing to their professional development through education on psychosocial issues and treatments.

• Participate in community outreach initiatives, including educational conferences and volunteer opportunities aimed at enhancing resources for cancer survivors.

Patient Population / Client Focus\n

Your primary focus will be on patients in a detox center context, which includes individuals struggling with substance use disorders and co-occurring mental health issues. You will help these patients navigate their emotional and psychological challenges, offering support during a critical juncture in their recovery journey.

Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution.

• Licensure as an Independent Clinical Social Worker (LICSW) in the state of Washington.
