Job Description

Dedicated nurse practitioners are needed to perform home health risk assessments for Medicare Advantage plan members throughout White County, including Monticello, Brookston, and Monon. This per diem opportunity offers flexible scheduling, allowing you to create a work routine that fits your life. As part of the role, youll travel to patients residences for annual assessments or post-discharge visits, with each evaluation averaging about 45 minutes.

Schedule:

• Flexible shifts available Monday through Sunday
• Create and customize your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history, relevant health conditions, and medication usage
• Educate patients on their chronic medical conditions and proper medication adherence
• Identify care gaps and offer actionable recommendations to help improve outcomes
• No requirement to formulate treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board certification through AANP or ANCC
• MSN degree earned from an accredited nursing program
• Current CPR (BLS) certification
• At least six months of experience as a Nurse Practitioner preferred
