This full-time position is primarily remote, allowing for flexible work arrangements while maintaining essential connection and collaboration with our interdisciplinary hospice care team. Key Responsibilities\n

• Conduct comprehensive evaluations and psychosocial assessments of patients and their families, understanding their unique situations and needs.
\n
• Develop and actively participate in the Individualized Family Service Plan (IFSP), tailoring approaches to each family’s circumstances.
\n
• Monitor patient outcomes and engage in team discussions to address shifting needs and ensure appropriate adjustments in care.
\n
• Provide emotional support to families, helping them navigate the hospice care process with sensitivity and empathy.
\n
• Collaborate with a multidisciplinary team, including nurses, physicians, and other healthcare professionals, to deliver holistic care.
\n
• Administer evaluations and assessments with a focus on evidence-based practices and natural environment methodologies.
\n
• Enhance family competencies by providing information and strategies that promote the developmental and emotional well-being of patients.
\n
• Ensure compliance with all relevant Federal and State health regulations and standards to safeguard patient health and safety.

Patient Population / Client Focus\n

In this role, you will work primarily with patients and families facing end-of-life decisions and challenges. Your expertise will guide supportive care for individuals dealing with life-limiting illnesses, focusing on improving their quality of life and facilitating meaningful transitions for both patients and their loved ones. Our facility specializes in addressing the needs of various populations, fostering a respectful and inclusive approach.

Qualifications\n

• Master’s Degree in Social Work or a Doctorate in Social Work (DSW).

• Current license as a Licensed Clinical Social Worker (LCSW), granted by the Department of Public Health.

• Valid driver’s license to facilitate travel as needed.
\n
• Strong communication skills and the ability to build rapport with diverse populations, including children, families, and team members.

Preferred Experience\n

• At least one year of experience working in pediatric early intervention or hospice settings, providing direct support to children and their families.

• Experience employing Natural Learning Environment Practices, enhancing developmental outcomes for children.

Compensation and Benefits\n
