Work Environment\n

This position offers a remote work setting, allowing you the flexibility to provide care and support to patients from various locations. While the typical work schedule is Monday to Friday, from 8:00 AM to 4:30 PM, occasional evening or weekend hours may be required based on client needs. As a member of an interdisciplinary team, you will collaborate with healthcare professionals, caregivers, and other social workers to ensure continuous care and support for clients. On-call availability may also be necessary to manage urgent situations.

Key Responsibilities\n

• Conduct in-depth assessments of the psychosocial needs of senior clients, taking into consideration their emotional, mental, and social circumstances.

• Develop personalized care plans in collaboration with clients and their families, tailoring support strategies to their individual goals and preferences.

• Provide counseling and emotional support to seniors and their families, assisting them in navigating life transitions such as grief, health changes, and more.

• Advocate for clients’ rights and facilitate access to community resources to ensure they receive the necessary support and services.

• Work collaboratively in interdisciplinary teams with nurses, caregivers, and various healthcare professionals to enhance the quality of care.

• Regularly monitor clients’ well-being through home visits, assessing living conditions and addressing any safety concerns.

• Facilitate effective communication and conflict resolution among family members involved in the care of the senior clients.
\n
• Educate clients and their families about available community resources, support services, and programs designed to enhance social engagement and quality of life.

• Maintain thorough and accurate documentation of client assessments, care plans, progress notes, and all relevant interactions.

• Stay current with social work policies, ethical standards, and best practices to ensure compliance and effective service delivery.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

We offer a competitive salary in the range of USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
