Key Responsibilities\n

• Conduct thorough and ongoing assessments to identify the unique needs and treatment requirements of your patients.

• Facilitate open communication among supervisors, direct care staff, families, guardians, and stakeholders to discuss patient progress and treatment plans.
\n
• Develop and present a schedule for appointments, home visits, and follow-up observations to supervisors, ensuring that treatment objectives are met efficiently.
\n
• Coordinate home visits as necessary to carry out in-home observations, engage in chart reviews, and conduct interviews that aid in comprehensive patient assessment.
\n
• Ensure the effective implementation of individualized treatment plans, adapting strategies as necessary to achieve optimal outcomes for clients.
\n
• Participate actively in interdisciplinary team meetings, sharing insights, and collaborating on treatment plans with fellow professionals.
\n
• Document patient progress through monthly and quarterly notes, providing clear updates on treatment effectiveness.
\n
• Organize and facilitate training for staff on program implementation and documentation standards to promote best practices.
\n
• Provide ongoing follow-up through interviews and home visits to support the continued growth and needs of clients.
\n
• Maintain meticulous records to ensure relevant information is accessible for authorized personnel and to support the therapeutic process.

• Act as a liaison within the mental health community, enhancing outreach and support for clients and facilitating program development.
\n
• Assist in creating and updating training materials to continually improve the quality of programs offered at the clinic.

Patient Population / Client Focus\n

The LPC will serve a diverse clientele, including adults and youth from various backgrounds, often facing challenges such as trauma, substance misuse, and family-related issues. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted license as a Professional Counselor in the state of Colorado.

• A minimum of two years of counseling experience in a clinical or outpatient setting is required.
\n
• Demonstrated proficiency in therapeutic modalities, with a preference for specialization in areas such as family therapy, couples therapy, trauma recovery, or substance use treatment.

• A strong understanding of legal, ethical, and professional standards in counseling.
\n
• Exceptional interpersonal skills, allowing the building of rapport with diverse client populations.

• Proficient in utilizing electronic medical record (EMR) software for documentation and case management.

Compensation and Benefits\n
