In this remote role, you will be part of a dedicated outpatient team that focuses on delivering comprehensive mental health services. You will utilize our advanced electronic health record (EHR) systems to monitor patient progress and maintain documentation efficiently, providing support for both you and your clients.

Key Responsibilities\n

• Conduct ongoing assessments and develop individualized treatment plans tailored to meet each patient's unique needs.

• Maintain clear and effective communication with supervisors, interdisciplinary team members, families, and caregivers to discuss client progress and modify treatment plans as necessary.

• Coordinate and schedule home visits, therapy sessions, and follow-up appointments to ensure comprehensive support and continuity of care.

• Facilitate interdisciplinary team meetings, documenting key discussions and decisions regarding treatment plans.
\n
• Prepare and submit monthly and quarterly progress notes to track the patient's development and response to therapeutic interventions.

• Guide and train staff members on the implementation of program plans and best practices for documentation processes.
\n
• Utilize evidence-based practices in individual and group therapy sessions, adapting strategies based on the patient's progress and feedback.

• Provide follow-up care through continuous engagement with patients, ensuring they receive ongoing support tailored to their needs.

You will work with a diverse clientele, including adults and youth, facing various mental health challenges. Qualifications\n

• A Master’s or Doctorate degree in a relevant field of mental health is required.
