Key Responsibilities\n

• Conduct comprehensive mental health assessments designed to identify the interplay between patients' psychiatric conditions and their medical history, using standardized assessment tools.

• Develop individualized treatment plans that incorporate both mental health and medical interventions, ensuring a holistic approach to care.

• Facilitate individual and group therapy sessions utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), tailored to the needs of each patient.

• Collaborate with physicians and other healthcare providers on a regular basis to discuss complex cases and coordinate care efforts for optimal patient outcomes.

• Provide crisis intervention services, including risk assessments and immediate support during mental health emergencies.

• Educate patients and their families on the impact of mental health on their overall health, offering them tools and resources for self-management of both mental and physical health conditions.

• Maintain thorough and confidential documentation compliant with HIPAA regulations and organizational standards, utilizing electronic health record (EHR) systems to track patient progress.

• Engage in ongoing professional development through case conferences and continuous training in integrated care methodologies.

Patient Population / Client Focus\n

In this role, you will primarily serve individuals grappling with co-occurring conditions, including mental health challenges exacerbated by chronic physical ailments such as autoimmune disorders and chronic pain conditions, as well as substance use disorders. You will play a crucial role in addressing not only their psychological needs but also supporting their medical treatment plans, thereby directly improving their quality of life.

Qualifications\n

• A master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) in California.

• Strong understanding of therapeutic modalities suited for individuals with medical complexities and experience in delivering integrated care.

Compensation and Benefits\n
