Work Environment

The position is onsite, with the unique opportunity to deliver care directly in the homes of our patients in Los Angeles, California. Key Responsibilities

As a Physical Therapist, you will:

• Conduct comprehensive evaluations to assess patients' physical capabilities and develop individualized treatment approaches.
• Implement evidence-based therapeutic strategies to improve mobility, strength, and overall functionality based on physicians' referrals.
• Utilize a variety of modalities, exercises, and manual therapy techniques to assist patients in recovering from orthopedic, neurologic, pediatric conditions, and sports injuries.
• Provide education and support to patients and their families regarding treatment plans, ensuring they are comfortable and well-informed.
• Monitor patient progress and modify treatment plans as necessary to meet their evolving needs.
• Document patient interactions and treatment outcomes meticulously to ensure continuity of care and support effective communication within the healthcare team.
• Collaborate with healthcare professionals including physicians and other therapists to enhance patient care quality and outcomes.

Patient Population / Client Focus

Your role will involve treating a wide range of patients, from those recovering from surgeries and injuries to individuals managing chronic conditions. You will work closely with orthopedic, pediatric, neurologic, and sports medicine patients, each presenting unique challenges and opportunities for recovery. Our goal is to keep our patients engaged, active, and mobile throughout their rehabilitation process, thereby fostering authentic relationships and demonstrating our commitment to their wellness.

Qualifications

• Graduation from an APTA or CAPTE-accredited Physical Therapy program.
• A Bachelor's degree in Physical Therapy is required.
• A valid state physical therapy license to practice in California.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Strong verbal and written communication skills to effectively engage with patients and their families.
• Proficiency in computer skills, including experience with electronic medical records.
• The ability to build strong, professional relationships with patients to help them exceed their functional goals and improve their quality of life.

Preferred Experience

• Prior experience in home care services or outpatient physical therapy settings is preferred.
• Experience treating a multifaceted patient population, especially in pediatric and geriatric cases.
• Familiarity with contemporary assessment tools and treatment methodologies will be an advantage.
