Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of clients and their families at the time of admission and periodically, or as clinically indicated, to evaluate the emotional, social, and physical needs influencing their mental health.

• Collaborate with interdisciplinary healthcare teams to create and implement effective psychosocial treatment plans, ensuring the coordination of referrals to specialists, social services, and community resources.
\n
• Utilize therapeutic techniques to assist patients and families in coping with the environmental and psychological issues associated with illness, enhancing their ability to manage challenges.
\n
• Independently manage a diverse caseload with varying clinical acuity, providing tailored interventions that address individual client needs.
\n
• Engage in proactive case finding to identify patients and families at risk for developing psychosocial issues, contributing insights during team conferences and interdisciplinary rounds.
\n
• Provide essential therapeutic services including supportive and grief counseling as well as crisis intervention, all aimed at facilitating patients' adjustment to their diagnoses and changes in care requirements.

• Deliver psychoeducation to clients and families, aiding their understanding of conditions, treatments, and prognosis while guiding them through transitions from youth to adult services.

• Participate in quality assurance initiatives aimed at enhancing social work services and addressing the evolving needs of our patient population.

• Formulate mental health diagnoses to inform treatment planning and deliver effective therapeutic interventions.

• Supervise clinical work for MSWs and student interns, ensuring adherence to professional standards while fostering their development.

• Maintain accurate electronic medical records in line with organizational policies, providing thorough documentation of client interactions and treatment progress.

This position serves a wide variety of patients, ranging from children and adolescents to adults and geriatric clients. Qualifications\n

• Licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) is required.
