OnTrack Physical Therapy & Wellness is seeking a dedicated and passionate Physical Therapist (PT) to join our team in beautiful Boca Grande, Florida. In this full-time position, you will play a vital role in our Orthopedic Outpatient Unit by delivering evidence-based care to improve the lives of patients suffering from a variety of orthopedic conditions. Work Environment

The work setting is onsite at our state-of-the-art rehabilitation facility, where you will collaborate closely with other physical therapists, nurses, and healthcare providers. Operating hours are Monday through Friday from 8 AM to 5 PM, allowing for a satisfying work-life balance.

Key Responsibilities

• Conduct comprehensive evaluations of patients, including gathering medical history and performing specific assessments to devise tailored treatment plans that reflect the patient's needs and objectives.
• Implement treatment protocols based on evaluation findings, ensuring that all interventions align with best practices and adapt to the evolving healthcare landscape.
• Document patient care activities meticulously, including writing legible daily encounter notes, interim progress summaries, and discharge evaluations that clearly depict the patient's course of therapy.
• Engage in collaborative care by consulting with other healthcare professionals and recognizing any changes in patients' conditions that necessitate adjustments in treatment plans or additional consultations.
• Provide thorough discharge planning and ensure patients understand their follow-up care, aiding in their recovery process post-therapy.
• Maintain compliance with state, local, and federal regulations and complete all required corporate and clinical training as necessary.
• Foster open communication with co-workers, physicians, patients, and their families, ensuring a respectful and supportive atmosphere.
• Participate in facility meetings and engage in professional development opportunities to remain current in the field.

Patient Population / Client Focus

In this role, you will primarily work with patients experiencing orthopedic concerns—such as post-surgical recovery, sports injuries, and chronic pain conditions. You will tailor treatment plans to meet the unique needs of each patient and focus on maximizing their outcomes through creative, evidence-based approaches.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current state licensure to practice physical therapy in Florida.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A strong commitment to ethical practice and patient-centered care.

Preferred Experience

While candidates with at least two years of relevant experience will be prioritized, we value enthusiasm and a strong willingness to learn. Experience in an outpatient orthopedic setting and familiarity with evidence-based treatment strategies will enhance your application.

Compensation and Benefits
