Work Environment

This position is based onsite within our home care facility, allowing physical therapists to engage directly with patients in a comfortable and familiar setting. Key Responsibilities

• Conduct thorough patient evaluations, which include reviewing medical history, performing detailed assessments, and formulating personalized treatment plans based on evaluation results.
• Communicate evaluation findings and treatment recommendations effectively to both patients and their families, actively involving them in goal-setting for recovery.
• Utilize evidence-based treatment protocols, adjusting interventions according to individual patient needs and the evolving healthcare landscape.
• Collaborate with multidisciplinary teams, documenting any changes in patient conditions, and making necessary modifications to treatment protocols when required.
• Oversee discharge planning, ensuring patients receive necessary follow-up care and maintaining timely and accurate documentation of patient care activities.
• Manage your schedule efficiently while meeting productivity expectations, arriving punctually to meetings and contributing to marketing initiatives that inform the community about our services.
• Participate in the continuous development and achievement of department-specific goals, upholding compliance with all relevant regulations and standards.
• Pursue ongoing education opportunities to enhance your professional skills and knowledge, while mentoring and guiding student interns as a clinical instructor.

Qualifications
