Position Overview

Trailbound Rehabilitation & Wellness Center is seeking a dedicated and experienced Physical Therapist (PT) to join our expanding team. Work Environment

This position is based onsite, primarily in the homes of our patients, allowing you to build meaningful relationships in a comfortable setting. You will work closely with a collaborative team of healthcare professionals, including occupational therapists and nursing staff, to ensure a streamlined therapeutic process. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical history, performing physical assessments, and identifying rehabilitation needs. Develop individualized treatment plans that reflect your insights and the unique requirements of each patient.

• Implement therapeutic interventions based on the latest evidence-based practices tailored to meet the needs of the patient, accounting for their unique health status and personal goals.

• Maintain clear, accurate, and timely documentation of all patient interactions, assessments, and progress notes in accordance with state and federal regulations. This includes creating comprehensive discharge plans that outline patient goals and interim progress summaries.

• Effectively communicate with other healthcare professionals to ensure cohesive care and monitor any significant changes in the patients' conditions that may affect their rehabilitation plans.

• Engage patients and their families in therapeutic processes, providing education and support to encourage adherence to treatment plans, ultimately enabling them to take an active role in their recovery.

• Participate in continual training and development sessions to stay abreast of advancements in the field and maintain compliance with local, state, and federal standards.

• Attend regular facility meetings to contribute to discussions regarding clinical protocols, patient feedback, and team dynamics, enhancing collaboration and improving service delivery.

Patient Population / Client Focus

As a Physical Therapist, you will primarily work with patients requiring home care services, focusing on orthopedic conditions. The diverse range of patients includes those recovering from surgeries, accidents, or conditions related to chronic pain. Your contributions will help these individuals regain independence, mobility, and confidence within the comfort of their homes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required; a Doctorate in Physical Therapy (DPT) is preferred.

• A valid license to practice as a Physical Therapist in Virginia is mandatory.

• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

While a minimum of 2 years of relevant clinical experience is required, we value candidates with strong backgrounds in home care settings. Compensation and Benefits
