Key Responsibilities\n

• Client Assessment: Conduct thorough assessments for clients, developing personalized treatment plans tailored to their unique needs.

• Treatment Planning: Collaborate with clinical teams to devise pre-crisis and crisis plans, risk assessments, positive behavior support plans, and medication treatment plans as needed.

• Cross-Functional Collaboration: Work alongside various programs to support the implementation of individualized care strategies and engage in a collective approach to client welfare.

• Community Liaison: Act as a liaison with local service systems and state agencies to facilitate access to necessary resources and support networks.

• Community Engagement: Establish and maintain relationships with local, state, and federal agencies to build partnerships and secure potential funding sources, aligning with our mission of community service.

• Professional Development: Commit to continuous personal and professional growth through attendance at workshops, conferences, and relevant community meetings.
