Key Responsibilities\n

• Conduct psychosocial assessments and develop tailored treatment plans that meet the unique needs of each client.

• Diagnose and treat diverse mental, emotional, and behavioral disorders, making timely adjustments to care plans in response to changes in client status.

• Facilitate effective referrals to community resources, treatment programs, support groups, and additional mental health providers to promote holistic well-being.

• Work collaboratively within a multi-disciplinary team to create comprehensive discharge plans, ensuring continuity of care after treatment concludes.

• Adhere to all relevant federal, state, and clinical standards, acting within your professional scope as defined by the Mental Health Practice Act.

• Engage in regular communication with patients, families, healthcare providers, and outside agencies to guarantee timely service delivery and to mitigate any issues or delays in care.

• Conduct social work consultations, psychosocial assessments, and provide appropriate interventions during crises or periods of bereavement.

• Advocate on behalf of your clients, ensuring they receive necessary support and that their voices are heard within multi-disciplinary care teams.

• Identify patients at risk for potential delays in care or discharge and coordinate with relevant parties to facilitate optimal solutions.

Qualifications\n

• A Master’s Degree in Social Work or a related field from an accredited institution, with all credentials verified prior to hiring.

Compensation and Benefits\n

The compensation for this position ranges from USD 90,000 to 100,000 annually, commensurate with experience and qualifications.
