Work Environment

This position is primarily an onsite role within the home care setting. Our physical therapists work directly in patients' homes, providing a unique and impactful approach to rehabilitation. Key Responsibilities

• Thoroughly reviewing each patient's medical history and current condition to create personalized treatment plans.
• Consulting directly with patients to assess their physical symptoms and establish therapeutic goals.
• Diagnosing movement dysfunction and developing comprehensive treatment strategies tailored to each individual's needs.
• Instructing patients on therapeutic exercise techniques, ensuring they can perform them correctly and safely.
• Administering manual therapies, such as massage and mobilization, to facilitate healing.
• Assisting patients with mobility devices, such as wheelchairs and walkers, to maximize independence.
• Maintaining accurate and up-to-date patient records, tracking progress, and setting achievable goals.
• Providing guidance to patients and their families on in-home exercises and treatment options.
• Collaborating with other healthcare professionals to ensure integrated and comprehensive patient care.

Qualifications
