Work Environment\n

This position offers a full-time schedule from Monday to Friday, with typical hours from 8:00 AM to 4:30 PM. While the convenience of remote work is available, occasional evening or weekend hours may be required depending on client needs. Flexibility is necessary for accommodating visits to ensure continuous and effective care delivery. Additionally, on-call availability may be required to address urgent situations or client concerns that arise outside of regular hours.

Key Responsibilities\n

• Conduct detailed psychosocial assessments for seniors to understand emotional, social, and mental health needs, considering vital life transitions such as loss, health deterioration, or family changes.

• Develop and implement personalized care plans in collaboration with clients and their families, ensuring that goals align with their values and preferences.

• Provide compassionate counseling and emotional support to seniors and their families, equipping them with coping strategies and resources to navigate challenges.

• Advocate for clients' rights, helping them access community resources such as healthcare services, support groups, and financial assistance programs.

• Work alongside an interdisciplinary team of healthcare professionals, including nurses and caregivers, to enhance the holistic care provided to our clients.

• Monitor clients' well-being through regular check-in visits, addressing any immediate concerns regarding their living conditions or health status.

• Facilitate effective communication among family members involved in the care process to resolve conflicts and improve collaboration.

• Educate seniors and their families on community resources and social engagement opportunities that can enhance quality of life.

• Maintain thorough and accurate documentation of client interactions, assessments, and care plans using electronic health record (EHR) systems to track progress and ensure continuity of care.

• Stay abreast of current social work practices, policies, and ethical guidelines to maintain compliance and enhance service delivery.

This role focuses primarily on serving the senior population, including those dealing with chronic illnesses, cognitive decline, mental health issues, or transitioning to different living arrangements. You will build trusting relationships with clients and their families, understanding their unique situations and providing tailored support that encourages independence and enhances their quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current licensure as a Licensed Master Social Worker (LMSW) in the state of Colorado.

Compensation and Benefits\n
