This position is designed to be entirely remote, providing flexibility in your work life while enabling you to engage with patients and their families from the comfort of your own home. Despite the remote nature of the role, you will be a critical part of our hospice care team, collaborating with other healthcare professionals through virtual meetings and tools to ensure that every patient receives comprehensive, coordinated care.

Key Responsibilities\n

• Conduct thorough mental health assessments, including psychosocial evaluations and risk assessments to better understand patients’ needs.

• Work closely with patients to craft individualized treatment plans that acknowledge their specific emotional needs and cultural backgrounds.

• Utilize a variety of evidence-based therapeutic techniques such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT) in one-on-one and group therapy sessions.

• Provide immediate crisis intervention for patients experiencing acute distress, coordinating care with medical teams to ensure consistent support.

• Maintain meticulous clinical documentation including treatment plans and progress notes in alignment with our policies and healthcare regulations.

• Educate patients and families about mental health management, equipping them with the tools and understanding necessary to make informed decisions.

• Collaborate with multidisciplinary teams, including nurses, doctors, and case managers, to ensure holistic care delivery for all patients.

• Evaluate treatment effectiveness through regular assessments, adjusting care strategies as needed based on patients’ evolving conditions.

• Stay abreast of advancements in mental health practices, integrating new knowledge into your therapeutic approach.

Patient Population / Client Focus\n

The patients you will support are individuals with terminal illnesses, along with their families who may be experiencing high levels of emotional distress and difficulty in coping with end-of-life issues. Your role as a social worker is crucial in providing psychological support through discussions around grief, loss, and acceptance, allowing families to find peace during challenging times.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution is required.

• Active State licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
