Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to determine their developmental needs.

• Participate actively in the Individualized Family Service Plan (IFSP) development, working collaboratively to tailor interventions that meet family goals.

• Monitor and document progress, adjusting strategies as necessary to maximize developmental outcomes.

• Provide evidence-based support to families, using a coaching model to empower them in enhancing their child's development.

• Facilitate regular communication with team members to ensure coordinated and patient-centered care.

• Comply with all relevant Federal and State regulations and standards to safeguard the well-being of clients and staff.

Patient Population / Client Focus\n

This role will focus primarily on infants and toddlers under three years of age who exhibit disabilities or delays in key developmental areas. You will be tasked with supporting these youngest clients and their families as they navigate the challenges of early intervention services. Your actions will significantly impact their developmental trajectory, equipping families with tools and knowledge to foster effective growth.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) through the Department of Public Health.

• A valid driver’s license is required for potential client home visits.
