Key Responsibilities\n

• Quickly establishing rapport and maintaining effective communication with patients, their family members, and the treatment team to foster trust and understanding.

• Conducting brief, evidence-based screenings to assess mental health needs, including evaluations for depression, anxiety, and substance use, tailoring approaches to suit the individual patient.

• Managing a case load of patients, providing timely follow-up interventions that address their specific needs and health goals.

• Utilizing electronic medical records (EMR) to document patient encounters, ensuring all entries are complete within 48 hours to support integrated healthcare practices.

• Designing and implementing comprehensive care plans that connect patients to a range of essential services while ensuring the confidentiality and privacy of patient information are maintained.

• Collaborating in quality improvement initiatives, case conferences, and departmental meetings to refine processes and enhance patient outcomes.
\n
• Identifying community resources that align with patient interests and goals, actively promoting linkage to professional services, educational programs, and support groups.
\n
• Responding promptly to inquiries from patients and family members regarding appointments or concerns about care.
\n
• Assisting patients in navigating the psychiatric medication process by coordinating with BCHC’s consulting psychiatrist.

• Performing other essential duties that contribute to the overall effectiveness of the hospice care team.

Patient Population / Client Focus\n

Your primary focus will be on hospice patients who are confronting complex emotional and psychological needs during a critical phase of their lives. You will work closely with diverse populations, including individuals from underserved communities, utilizing a trauma-informed approach to address a wide range of issues related to grief, loss, and mental health. Your expertise will guide patients and their families through this sensitive time, ensuring they receive compassionate support tailored to their unique circumstances.

Qualifications\n

• Master's degree in a related behavioral health field.

• Current LCSW licensure is mandatory.

• CPR or BLS certification.
