Work Environment

This position operates primarily in a home care setting, where flexibility and patient interaction are paramount. You will be required to travel to various patient locations throughout Jacksonville, utilizing your skills to assess and implement treatment strategies effectively. Key Responsibilities

• Evaluate and treat patients within their homes, adapting to a variety of settings and circumstances.
• Develop individualized treatment plans based on comprehensive patient assessments, integrating various therapeutic techniques such as exercises, manual therapy, and specialized equipment.
• Utilize advanced modalities and technologies to alleviate pain and improve functional mobility.
• Track and document patient progress through accurate record-keeping and effective communication with both patients and healthcare team members.
• Conduct regular re-evaluations to modify treatment plans based on patient responses and recovery milestones.
• Guide patients and caregivers in utilizing assistive devices and self-care strategies to promote independence.
• Educate patients, families, and within the healthcare team about effective therapeutic interventions and home exercise programs.
• Stay current with the latest research and developments in physical therapy by attending workshops and participating in professional societies.

Patient Population / Client Focus

In this role, you will work with diverse patient populations that encompass all ages and conditions, including but not limited to post-operative recovery, stroke rehabilitation, orthopedic injuries, and chronic illness management. Your efforts will focus on enhancing their quality of life and ensuring that they can actively participate in their daily activities with confidence and safety.

Qualifications

• Must possess a Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Currently licensed as a Physical Therapist in the state of Florida.
• Exceptional communication skills, both verbal and written, to ensure clear interactions with patients and the healthcare team.
• Proficiency in computer skills, including word processing and electronic health records.
• Ability to adapt to changing conditions and patient needs in a home care setting.

Preferred Experience

• While prior professional experience is not required, candidates with clinical experience or internships in home care settings will be favored.
• Interest or specialization in pediatric or geriatric physical therapy is a plus.

Compensation and Benefits
