Work Environment

This position will be 100% onsite, as you will provide essential physical therapy services directly within clients' homes. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions using diagnostic tools to assess muscle, nerve, joint function, and activities of daily living.
• Design and implement individualized therapy plans to address mobility issues and rehabilitative needs, ensuring they align with the prescribed medical treatment.
• Guide and supervise Physical Therapy Assistants to guarantee effective execution of therapy sessions while ensuring compliance with the established plan of care.
• Administer a range of therapeutic techniques including manual therapy, exercise regimens, traction, and various physical agents to promote recovery and improve functional abilities.
• Regularly assess patient progress and modify treatment plans based on observed outcomes and feedback from both patients and caregivers.
• Maintain thorough clinical documentation, including evaluations, progress notes, and discharge summaries, in compliance with relevant regulations and organizational standards.
• Engage with patients and their families to provide education on therapy techniques, equipment use, and self-management strategies.
• Collaborate with other healthcare professionals in interdisciplinary team meetings to discuss patient progress and co-create comprehensive treatment approaches.

Patient Population / Client Focus

Our primary focus is on patients who require personalized care in their homes, including those recovering from surgery, managing chronic illnesses, or experiencing mobility challenges. You will work closely with geriatric patients, individuals with disabilities, and those requiring specialized rehabilitation services, tailoring your approach to meet each patient's unique circumstances and goals.

Qualifications

• Required: A Doctorate in Physical Therapy (DPT) from an accredited program.
• Possession of a valid and active Physical Therapist license in Florida, free from sanctions or disciplinary actions.
• A strong foundation in manual therapy techniques and a proven track record of developing effective exercise programs.
• Proficiency in using Electronic Medical Records (EMR) systems, particularly Web PT, to ensure accurate documentation and patient tracking.
• Ongoing commitment to staying up-to-date on physical therapy advancements and best practices.

Preferred Experience

• Experience in home health settings, particularly with elderly and physically challenged clients.
• Demonstrated ability to collaborate effectively within an interdisciplinary team.
• Prior involvement in developing care plans and leading patient education initiatives.

Compensation and Benefits

The salary for this position ranges from $100,000 to $120,000 annually, commensurate with experience and qualifications.
