Licensed Full-Time Physical Therapist for Home Care Services

Work Environment

This role is based onsite, providing in-home care to patients across Jacksonville. Our team will work collaboratively with you to ensure that our patients receive the best possible service in a familiar and comfortable setting. You will have the opportunity to develop strong, trusting relationships with your patients as you support them through their rehabilitation journey.

Key Responsibilities

• Conduct thorough assessments to establish patient care goals and develop individualized treatment plans.
• Provide evidence-based therapeutic interventions designed to enhance range of motion and improve mobility.
• Administer specialized therapies that minimize reliance on medications and offer non-surgical options.
• Educate and motivate patients to adhere to their treatment plans, emphasizing exercise and nutrition to achieve their health objectives.
• Utilize manual therapy techniques and specialized equipment to facilitate recovery effectively.
• Design customized fitness and wellness programs specific to each patient's physical capabilities and goals.
• Document patient progress accurately and regularly, maintaining compliance with healthcare regulations and standards.
• Collaborate closely with other healthcare providers to discuss patient cases, share insights, and improve coordinated care.
• Perform regular evaluations to monitor progress, adjust treatment protocols as necessary, and communicate findings with patients and their families.
• Engage in continuous education and training to remain current in the field and introduce new strategies to enhance patient care.

Patient Population / Client Focus

You will primarily work with patients recovering from orthopedic surgeries, injuries, and chronic pain conditions. This role places a strong emphasis on rehabilitation for adult populations. Understanding the unique challenges faced by each patient will be essential to developing effective treatment strategies that not only address their physical impairments but also consider their emotional and psychological well-being.

Qualifications

• A Master's or Doctorate degree in Physical Therapy (MPT or DPT) from an accredited institution.
• A valid Physical Therapist license to practice in the state of Florida.
• No record of disciplinary actions or board certifications affecting the license.
• Proficient in manual therapy techniques and exercise program design.
• Experienced with Electronic Medical Records (EMR) software, preferably WebPT.

Preferred Experience

• A minimum of two years of experience working as a Physical Therapist, particularly in orthopedic outpatient settings, is preferred.
• Demonstrated ability to develop and implement individualized treatment plans effectively.
• Hands-on experience in a home care setting is highly valued.

Compensation and Benefits
