Work Environment

In this primarily onsite role, you will work directly in the homes of our patients across the Jacksonville area. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions and functional abilities, taking into account their unique health histories and rehabilitation objectives.
• Design and implement comprehensive, individualized treatment plans tailored to each patient's specific needs and goals, utilizing evidence-based practices.
• Deliver hands-on therapy, which includes manual therapy techniques, therapeutic exercises, and appropriate modalities to maximize patient recovery.
• Educate patients and their families on effective home exercise programs, injury prevention strategies, and lifestyle modifications that support ongoing health and wellness.
• Continuously monitor and document patient progress using state-of-the-art electronic health record systems, making timely adjustments to treatment plans as required.
• Collaborate actively with an interdisciplinary team of healthcare professionals to ensure coordinated and comprehensive patient care; engage in case consultations when necessary.
• Maintain meticulous and compliant medical records, ensuring that all documentation adheres to local, state, and Medicare regulations.
• Stay informed of the latest developments in physical therapy methodologies and best practices, integrating new techniques into your patient care routine.

Patient Population / Client Focus

The Physical Therapist will primarily work with adult and elderly patients who may be recovering from surgery, managing chronic pain, or experiencing mobility challenges due to various orthopedic conditions. By developing rapport and trust, you will play a crucial role in empowering patients to become active participants in their recovery journey.

Qualifications

• A Doctorate or Master's degree in Physical Therapy from an accredited institution.
• Current and valid Physical Therapist license in the state of Florida.
• A minimum of three years of clinical experience in physical therapy or a closely related field, demonstrating expertise in patient assessment and treatment interventions.
• Strong clinical skills, particularly in performing and evaluating a broad range of therapeutic techniques.
• Excellent communication and interpersonal skills, enabling effective interactions with patients and their families.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary ranging from $90,000 to $120,000 annually, commensurate with experience and qualifications.
