Work Environment

This position is based in Jacksonville, Florida, and involves onsite patient visits directly in their homes. Our working environment promotes collaboration, with consistent communication among team members, including two experienced physical therapists and one Physical Therapy Assistant (PTA). The freedom to work closely with patients in their own spaces allows for personalized care and the ability to create meaningful connections.

Key Responsibilities

• Conduct thorough evaluations of clients to identify individual needs and establish personalized treatment goals.
• Administer effective physical therapy interventions with an emphasis on orthopedic conditions, ensuring a plan centered around improving mobility and reducing pain.
• Manage a case load of 2-3 patients per hour, organizing treatment plans that may include exercises, manual therapy, and specialized modalities.
• Monitor client progress and adapt treatment plans proactively based on ongoing assessments and observable outcomes.
• Create fitness and wellness programs, focusing on strength, flexibility, balance, and coordination to enhance client performance in daily activities.
• Collaborate with multidisciplinary teams, consulting with other health professionals to ensure cohesive care for patients.
• Document patient interactions and treatment outcomes in electronic medical records (EMR) systems, such as WebPT, ensuring compliance with healthcare standards.
• Provide patient education on lifestyle modifications and therapeutic exercises to empower clients in their recovery journey.

Patient Population / Client Focus

In this role, you will primarily work with adult and geriatric populations who are facing orthopedic challenges. Our clients often experience pain from injuries, surgeries, or chronic conditions, and your role will be pivotal in guiding them through rehabilitation processes, restoring function, and maintaining their independence.

Qualifications

• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Valid Florida Physical Therapist License without disciplinary actions.
• Proficiency in manual therapy techniques and foundational exercise principles.
• Familiarity with EMR software, such as WebPT, for documenting patient care services effectively.

Preferred Experience

• A minimum of 2 years of hands-on experience in orthopedic outpatient physical therapy.
• Demonstrated ability to create effective, individualized treatment plans based on patient assessment.
• Experience in home care settings is a plus but not mandatory.

Compensation and Benefits
