Active Mobility PT is seeking a dedicated and compassionate Licensed Orthopedic Physical Therapist to join our team in Jacksonville, Florida. Work Environment

The successful candidate will work primarily in patients' homes, delivering tailored physical therapy sessions that promote healing and recovery in a comfortable, familiar environment. This onsite approach allows therapists to observe and engage with patients in their daily living situations, enhancing the therapeutic experience. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing their physical condition and functional limitations to formulate effective treatment plans.
• Implement and adjust individualized therapy programs that address the unique needs of your patients, utilizing a variety of manual therapy techniques, therapeutic exercises, and equipment as necessary.
• Provide education and training for patients and caregivers to empower them to participate actively in the rehabilitation process.
• Utilize evidence-based practices and stay current with advancements in physical therapy to maximize patient outcomes.
• Collaborate with a multidisciplinary team to continuously improve treatment strategies and share insights that can enhance care quality.
• Maintain accurate and timely records of patient progress, treatment plans, and communication with other healthcare providers as required.
• Demonstrate excellent customer service by establishing and nurturing positive relationships with each patient, ensuring they feel valued and supported throughout their recovery journey.
• Meeting established utilization goals and contributing to the overall objectives of the organization.

Patient Population / Client Focus

You will work with a diverse range of patients, including those recovering from orthopedic injuries, surgeries, and chronic conditions. Qualifications

• Bachelor's Degree in Physical Therapy from an APTA or CAPTE-accredited program.
• Current State License as a Physical Therapist in Florida.
• American Heart Association Basic Life Support (BLS) Certification.
• Excellent verbal and written communication skills, demonstrating the ability to connect with patients, families, and colleagues.
• Proficiency in computer skills, including word processing and the use of electronic health records.

Preferred Experience

While a Bachelor's Degree is the minimum educational requirement, candidates with prior experience in home health or outpatient orthopedic settings will be prioritized. A demonstrated track record of establishing rapport with patients and a commitment to ongoing professional development is strongly encouraged.

Compensation and Benefits

The salary range for this position is competitive, ranging from USD 80,000 to 100,000 per year, commensurate with experience. In addition to salary, we offer a comprehensive benefits package that includes:
