Work Environment

This position is primarily focused on home care, which requires travel to patients' residences throughout Jacksonville. Key Responsibilities

• Conduct thorough physical therapy assessments to analyze patients' mobility, strength, and range of motion.
• Design and implement individualized treatment plans specific to each patient's rehabilitation goals and progress.
• Employ a combination of therapeutic interventions, including manual therapy, exercises, and functional training to enhance recovery.
• Utilize various modalities like ultrasound, electrical stimulation, and cryotherapy as deemed appropriate for treatment plans.
• Educate patients and their families on preventive strategies for injury and the importance of home exercise programs.
• Collaborate closely with physicians, occupational therapists, and other specialists to ensure comprehensive care and optimal patient outcomes.
• Maintain accurate, detailed documentation of patient progress and treatments in compliance with clinic protocols and healthcare regulations.

Patient Population / Client Focus

In this role, you will primarily work with adults and seniors who are recovering from orthopedic surgeries, sports injuries, or chronic musculoskeletal conditions. Your efforts will significantly impact their rehabilitation journey, promoting not only physical recovery but also emotional support through compassionate care.

Qualifications

• Education: Bachelor's Degree in Physical Therapy from an accredited program is required; a Doctor of Physical Therapy (DPT) is preferred.
• Licensure: Must hold a current and valid Physical Therapy license in the state of Florida.
• Skills: Demonstrated proficiency in manual therapy techniques and the application of evidence-based practices.
• Communication: Strong interpersonal and communication skills to effectively interact with patients, families, and team members.
