Flexible PRN schedule that fits your lifestyle and availability Meaningful one on one patient relationships Opportunity to work in home and community settings where real progress happens Collaborative interdisciplinary team environment Clinical autonomy and flexibility Continuing education and professional development opportunities A national leader in neuro rehabilitation serving 21 states and growing since 1995 What You’ll Do Provide psychosocial assessments and emotional support for patients and families Assist with coping strategies, adjustment, and community reintegration Connect patients with community resources and support services Collaborate with therapists, physicians, case managers, and caregivers to support treatment goals Advocate for patients throughout their rehabilitation journey Responsibilities Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families Sets measurable objectives that are formulated in conjunction with the rehab team Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives Qualifications Master's Degree in Clinical Social Work (LCSW) Current Licensed Clinical Social Worker Licensure in the state of practice Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis Current BLS (Basic Life Support) First Aid and CPR certification required Ability to lift 50 pounds Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures Duties require fine motor skills, visual acuity, and walking/ standing for extended periods Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times About Our Line Of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
