This position is fully remote, allowing you flexibility while you perform your duties. Although you will predominantly work from your home, you will be an integral part of our virtual hospice team, engaging with healthcare professionals, patients, and families via telehealth services. Key Responsibilities\n

• Provide effective, evidence-based clinical services to patients and families navigating the complexities of end-of-life care and emotional distress.

• Conduct thorough psychosocial assessments and develop individualized care plans that align with patients' needs and clinical goals.

• Facilitate ongoing therapy sessions, utilizing techniques such as cognitive behavioral therapy (CBT) and mindfulness, both in scheduled teletherapy and via other virtual communication methods.

• Monitor and advocate for patients' eligibility for various resources, ensuring they have access to necessary support services that enhance their quality of life.
\n
• Collaborate with physicians, nurses, and other healthcare providers to create an interdisciplinary approach to patient care, ensuring continuity and quality throughout their hospice experience.

• Educate families on coping strategies and available resources while fostering a supportive environment where concerns can be expressed freely.

Patient Population / Client Focus\n

In this role, you will primarily work with terminally ill patients and their families, providing critical psychological support during an emotionally sensitive period. You will focus on understanding the unique challenges faced by each individual, ensuring that their psychosocial needs are met comprehensively. This includes addressing the emotional, spiritual, and practical aspects associated with hospice care.

Qualifications\n

• Possession of a Bachelor’s degree in Social Work or related field, coupled with a current and valid LICSW license.

• Minimum of three years of experience in clinical social work, ideally with a focus on hospice, palliative care, or community health.

• Demonstrated expertise in utilizing crisis intervention skills and evidence-based therapeutic practices.

• Strong ability to establish rapport and foster relationships with diverse populations, treating all individuals with respect and dignity.

Preferred Experience\n

• Experience working effectively with vulnerable populations, particularly unhoused individuals or those coping with chronic health conditions.

• Familiarity with Medicaid, Medicare, and private insurance plans, along with the implications for patient care and funding.

• Knowledge of community resources available to support patients during hospice care, enhancing continuity of support beyond therapy sessions.

Compensation and Benefits\n

The compensation for this position ranges from $100,000 to $105,000 annually, reflecting your skills, experience, and dedication to providing high-quality care. In addition to competitive salary, we offer a comprehensive benefits package that includes:
