Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand patients' emotional, social, and financial needs.

• Provide individualized and group counseling, addressing adjustments, grief, and coping with complex health issues.

• Collaborate with interdisciplinary teams—including physicians, nurses, and psychologists—to develop comprehensive care plans aimed at optimizing patient outcomes.

• Coordinate with community organizations and resources to facilitate continuity of care post-discharge and ensure necessary support services are in place.

• Offer emotional support and education to patients and families, guiding them through treatment options and available resources.

• Maintain accurate and timely documentation of patient interactions in accordance with legal and organizational policies, ensuring compliance and quality assurance.

Patient Population / Client Focus\n

The LICSW will work primarily with patients in hospice care—individuals facing life-limiting illnesses and their families. This patient population requires specialized support, and your role will focus on providing empathetic counseling, facilitating discussions around end-of-life decisions, and ensuring that each patient receives dignified care aligned with their values and preferences.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.

Compensation and Benefits\n
