Key Responsibilities\n

• Conduct thorough mental health assessments and ongoing evaluations to understand and address the needs of clients effectively.

• Facilitate open communication among supervisors, colleagues, families, and guardians to ensure cohesive treatment plans.

• Design and maintain individualized treatment strategies, including detailed plans for appointments, home visits, and case management sessions aimed at tracking client progress.

• Organize and perform in-home visits to conduct observations and assessments that inform treatment decisions.

• Participate actively in Interdisciplinary Team Meetings to evaluate and improve clinic practices.
\n
• Complete and document monthly and quarterly progress notes to monitor therapeutic outcomes and adjust treatment strategies as necessary.

• Assist in developing training materials to enhance the skills of staff members in executing treatment plans and adhering to documentation requirements.

• Conduct follow-up evaluations and observations to ensure that ongoing client needs are met effectively.

• Maintain accurate and accessible client records to facilitate communication and care continuity within the healthcare team.

• Engage with the broader mental health community to promote initiatives and resources that support client growth.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work primarily with a diverse patient population, including both adults and youth facing mental health challenges such as anxiety, depression, trauma, and substance misuse. Qualifications\n
