Key Responsibilities\n

In your role as a Licensed Professional Counselor, you will be responsible for:

• Conducting ongoing assessments and therapeutic evaluations to tailor services to meet the individual needs of patients.

• Facilitating communication with supervisors, direct care staff, family members, and other stakeholders to discuss patient progress and adjust treatment plans as necessary.

• Creating and maintaining a tentative schedule for patient appointments, home visits, and observational work to effectively monitor treatment progress.

• Coordinating various home visits to conduct necessary in-home observations, chart reviews, and staff interviews, ensuring comprehensive data analysis to inform care.

• Implementing and regularly reviewing the program plan to ensure its effectiveness and relevance to patient needs.
\n
• Participating in interdisciplinary team meetings, contributing insights and strategies to optimize patient care outcomes.
\n
• Documenting monthly and quarterly progress notes that accurately reflect patient advancement, challenges, and necessary adjustments.
\n
• Organizing and leading training sessions for staff to enhance their competency in executing program plans and documentation procedures.
\n
• Providing follow-up interviews and ongoing support through home visits, ensuring continuity of care for all individuals.

• Maintaining meticulous records to guarantee that information remains accessible and accurate for all involved parties.

• Serving as a liaison with the broader mental health community to support development goals and enhance the clinic's outreach and effectiveness.

• Assisting in the creation and refinement of training materials aimed at improving program implementation for diverse populations.

Patient Population / Client Focus\n

Your work will encompass a diverse client focus, including both adults and youth experiencing a variety of mental health challenges. The ability to adapt your therapeutic approach to suit the unique needs of each individual, whether it's through family therapy, trauma-informed care, or substance misuse interventions, is crucial in fostering healthy outcomes.

Qualifications\n

To be considered for this role, you must possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• An active and unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey.

• A minimum of two years of direct counseling experience, preferably in a clinical setting.

• A strong understanding of legal and ethical standards governing mental health practice.

• Exceptional interpersonal and communication skills to build rapport with a diverse patient population.

Preferred Experience\n

Compensation and Benefits\n
