Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to meet the diverse needs of patients in your care.

• Establish and maintain clear communication channels with supervisors, family members, and relevant parties to discuss patient progress and development.

• Draft and implement client care plans that detail appointments, in-home visits, and sessions, ensuring effective tracking of patient progress.
\n
• Conduct home visits and in-home observations, collaborating with staff to gather insights through chart reviews, interviews, and data analysis.

• Ensure thorough execution of program strategies while participating in interdisciplinary team meetings to support collaborative care.
\n
• Document monthly and quarterly progress notes, maintaining accuracy to uphold compliance and care standards.
\n
• Coordinate training sessions to enhance the capabilities of staff and ensure competency in paperwork related to the program plan.
\n
• Carry out follow-up interviews and evaluations to ascertain the ongoing needs of individuals and adapt treatment plans accordingly.

• Keep detailed records that are accessible to authorized personnel and contribute to organizational transparency.

• Support the clinic’s outreach efforts by engaging with the community and connecting them with mental health resources.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including both adults and youth facing various mental health challenges. Your focus will be on the delivery of evidence-based therapeutic approaches tailored to meet the specific needs of each individual. Special emphasis on techniques related to family therapy, couples therapy, substance use, and trauma-informed care is encouraged.

Qualifications\n

• A Master’s degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Georgia.

• A minimum of two years of experience in a clinical counseling setting, demonstrating the ability to manage diverse patient cases.
