Key Responsibilities\n

As a Licensed Professional Counselor at Intellect Capacity Mental Clinic, you will be responsible for:

• Maintaining ongoing communication with healthcare teams, family members, and guardians to monitor and assess the development of individuals in care.

• Developing comprehensive care plans that include scheduling appointments, conducting home visits, and undertaking various assessments to track patient progress.

• Facilitating interdisciplinary team meetings to discuss patient outcomes and collaboratively adjust treatment strategies as necessary.

• Documenting monthly and quarterly progress notes to ensure accountability and clarity in patient care efforts.

• Providing ongoing follow-up, including additional interviews and home visits, to ensure that all patient needs are addressed effectively.

• Building community relationships and acting as a liaison between the clinic and the broader mental health network to promote community wellness.

• Contributing to the development of training materials and facilitation of workshops for staff to enhance service delivery.

Patient Population / Client Focus\n

This role will involve working with a diverse range of individuals, including both adults and youth, from various backgrounds. Qualifications\n

To be considered for this position, candidates must possess the following qualifications:
