Key Responsibilities\n

• Conduct thorough assessments of patients’ biopsychosocial needs to inform individualized treatment plans, ensuring a comprehensive understanding of each patient’s unique situation.

• Provide case management support including short-term counseling and crisis intervention to help patients navigate challenges and enhance their coping mechanisms.

• Assist patients in obtaining essential benefits by guiding them through the completion of necessary forms for entitlements and supplemental insurance, ensuring they receive appropriate support and resources.

• Help patients access prescription cost assistance through various indigent programs and connect them with community resources aimed at alleviating financial burdens related to healthcare.

• Act as a patient advocate during interactions with community agencies, ensuring their needs are effectively communicated and addressed.

• Maintain accurate patient records, prepare detailed reports, and document all interactions according to clinic policies and legal requirements to ensure high-quality care standards.

• Coordinate transportation assistance for patients when necessary, facilitating access to vital mental health services and ensuring their appointment attendance.

• Participate in regular clinical and team meetings to discuss ongoing patient cases, share best practices, and contribute to continuous improvement efforts within the organization.

Qualifications\n
