Key Responsibilities

• Conduct comprehensive health assessments, including physical exams and medical history reviews, to inform patient care strategies.
• Develop and implement individualized care plans aimed at health promotion, disease prevention, and management of acute and chronic illnesses.
• Perform diagnostic tests, interpret results, and adjust treatment plans as necessary, ensuring that patients receive evidence-based care.
• Provide patient education on health maintenance and risk factor modification, empowering patients to make informed decisions about their health.
• Coordinate patient care with other healthcare providers, specialists, and community resources to ensure a holistic approach to patient wellness.
• Document all patient interactions and care plans in compliance with established protocols, ensuring accurate and timely medical records.

Qualifications

Compensation and Benefits
