Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Millrift, Pennsylvania. Job Description & Requirements Specialty: Home Health Discipline: RN Duration: Ongoing Hours: 40 hours per week, 8-hour shift, full-time, staff employment type. Responsibilities Lead and collaborate to provide management and delivery of patient care plans as a case manager. Provide patient care and case management for patients in Matamoras, PA, and surrounding areas. Travel to patient locations, spending approximately 20% of the day driving. Ensure timely charting by meeting critical deadlines. Self-manage time and schedule to meet client needs. Qualifications Active RN license in the state of practice. Graduate from an approved school of practical nursing. One year of RN experience and clinical competence in delivering quality patient care. Current driver’s license. Demonstrated experience with tablets, mobile phones, and EMR software.
