Key Responsibilities\n

As a Licensed Clinical Professional Counselor, you will undertake a variety of essential responsibilities, including:

• Collaborating with primary care providers to recognize and manage mental health and psychosocial issues among patients.
\n
• Conducting thorough patient intakes, focusing on diagnostic assessments and functional evaluations, followed by providing tailored recommendations to the primary care team.
\n
• Facilitating brief, focused therapeutic interventions for patients in need of immediate mental health support.
\n
• Identifying patients who would benefit from specialty mental health referrals and coordinating these referrals effectively.
\n
• Initiating follow-up communications with patients to monitor progress and adjust treatment approaches as necessary.
\n
• Creating and implementing relapse prevention plans for clients, aimed at helping them maintain stability in their mental health.
\n
• Detecting at-risk patients and developing proactive strategies to prevent deterioration of their condition.
\n
• Documenting patient progress and diagnostic information meticulously in treatment charts to ensure continuity of care.
\n
• Providing education and self-management skills to patients, empowering them to take an active role in their recovery journey.

• Maintaining open communication with the primary care team regarding patient needs and treatment progress.

• Consulting with clinic management and peers on behavioral health service delivery, with a focus on program evaluation and outcome improvement.

Patient Population / Client Focus\n

Your work will primarily focus on a diverse patient population exhibiting a range of mental health challenges. From children to adults, you will address their emotional and psychological needs, ensuring that every client receives personalized care that aligns with our agency’s commitment to confidentiality and ethical practice.

Qualifications\n

To be considered for the position, applicants must possess the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling.
\n
• Current licensure as a Licensed Clinical Professional Counselor (LCPC) in Maryland.
\n
• Solid understanding of mental health assessment, treatment planning, and various therapeutic modalities.

• Competence in addressing mental health conditions as per DSM-IV diagnoses, along with knowledge in family therapy and behavioral interventions.

• Excellent interpersonal skills and the ability to establish effective relationships with clients, families, and colleagues.
\n
• Strong documentation skills, ensuring accurate and timely updates to patient records.
\n
• Ability to work independently in a remote setting while complying with established guidelines and productivity targets.

Preferred Experience\n

While not mandatory, the following experiences are preferred:

• Experience working within Federally Qualified Health Centers (FQHC).
\n
• Proficiency in Spanish, in addition to English, is advantageous but not required.

• Background in interdisciplinary team collaboration and cooperative care models.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 87,000 to USD 105,000 annually, commensurate with experience and qualifications.
