Full-Time Home Care Physical Therapist

Limber Physical Therapy is seeking a dedicated and qualified Physical Therapist to join our team in Bradenton, Florida. This full-time role focuses on providing high-quality physical therapy services to patients in their homes. As a Physical Therapist, you will be responsible for assessing, planning, and implementing individualized treatment programs that empower patients to reclaim their movement and enhance their quality of life.

Work Environment

This position is offered in an onsite home care setting, where you will directly engage with patients in their own environments. Working primarily Monday to Friday for 40 hours a week, you will have a unique opportunity to witness and facilitate healing in a familiar space for each patient. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to determine their physical therapy needs.
• Develop and implement customized treatment plans that focus on enabling patients to achieve their recovery goals.
• Provide expert therapeutic care in patients' homes, utilizing techniques and procedures that align with their physician's treatment plan.
• Educate and train patients and their caregivers on exercise techniques, safety measures, and lifestyle modifications.
• Document patient progress and respond to any changes in patient conditions promptly.
• Work collaboratively with a multidisciplinary team to enhance patient outcomes through evidence-based practice.
• Deliver superior customer service to build trust and rapport with each patient and their families.
• Monitor therapy metrics and meet utilization goals as part of broader business objectives.

Patient Population / Client Focus

As a Physical Therapist at Limber Physical Therapy, you will have the chance to work with diverse patient populations across various specialties, including orthopedic, pediatric, neurologic, and sports medicine cases. Qualifications

• Must hold a Bachelor's degree in Physical Therapy from an APTA or CAPTE-approved program.
• A valid state license to practice physical therapy in Florida.
• Current Basic Life Support (BLS) Certification from the American Heart Association.
• Proficiency in written and verbal communication, as well as relevant computer and word processing skills.
• A commitment to establishing strong, impactful relationships with patients to help them achieve their functional goals.

Preferred Experience

• A minimum of 2 years of clinical experience in physical therapy, particularly in home care settings.
• Skills in utilizing various therapeutic modalities and treatments tailored to different patient needs.
• Experience working with a multidisciplinary team to maximize patient rehabilitation outcomes.

Compensation and Benefits
