Key Responsibilities\n

• Conduct ongoing assessments to determine and address the mental health needs of patients, employing evidence-based practices and ethical standards.

• Facilitate communication among supervisors, family members, and other stakeholders regarding the progress and treatment plans of individuals, ensuring a cohesive approach to care.
\n
• Develop and propose schedules for appointments, home visits, and necessary observations to continually monitor patient progress and treatment efficacy.
\n
• Coordinate and execute in-home visits, ensuring thorough chart reviews and data analysis to enhance at-home treatment initiatives.
\n
• Implement program plans effectively, ensuring they meet the unique needs of each patient served.
\n
• Engage in Interdisciplinary Team Meetings and other relevant gatherings to discuss patient care and ongoing clinical strategies.
\n
• Prepare and complete monthly and quarterly progress notes that reflect patient advancements and concerns.
\n
• Lead training initiatives aimed at certifying staff and team members in the proper implementation of program plans and documentation requirements.
\n
• Provide ongoing support through follow-up interviews and home visits to ensure comprehensive and sustained care.
\n
• Maintain accurate and organized records to support clinicians, patients, and collaborating agencies.
\n
• Act as a liaison with the public and mental health community, fostering connections to support developmental goals within the framework of our mission.

• Assist in developing training materials and procedures to improve the quality of programs available for individuals seeking mental health support.

This role is crucial in serving a diverse patient population, including adults and youth from varying backgrounds. Our counselors are tasked with addressing an array of mental health challenges, such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related discipline.

• Active and unrestricted licensure as a Professional Counselor (LPC) within the state of Arizona.

• Minimum of two years of relevant experience, with a preference for background in a clinical setting.

• A strong specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly desirable.

• Deep understanding of both legal and ethical standards governing mental health practice.

• Exceptional interpersonal and communication skills, capable of establishing rapport with diverse clientele.

• Demonstrated ability to work collaboratively with a multidisciplinary healthcare team.

• Familiarity with Electronic Medical Records (EMR) systems and other relevant technologies is essential.
