Work Environment

The role of the Physical Therapist at our Jacksonville location will be onsite within our well-equipped rehabilitation facility. Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations, developing and implementing individualized treatment plans aimed at achieving specific rehabilitation goals.
• Provide direct physical therapy services, including therapeutic exercises, manual therapy, and functional training, in alignment with physician-prescribed care.
• Oversee the implementation of treatment plans by physical therapy assistants, ensuring the highest quality of care is delivered according to established protocols.
• Monitor and record patients' progress, making necessary adjustments to treatment plans to reflect evolving needs and coordinating with the wider health care team as needed.
• Educate patients and their families on rehabilitation techniques and exercises to maximize independence and safety in daily activities.
• Maintain accurate documentation of clinical records, including evaluations, progress notes, and discharge summaries, ensuring compliance with regulatory standards.
• Participate in interdisciplinary team meetings to discuss patient progress and modify treatment plans as required.
• Provide recommendations for home modifications or adaptive devices to enhance patients' ability to perform daily tasks.
• Stay current with advancements in physical therapy practices and contribute to continuous quality improvement initiatives within the organization.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from surgeries, managing chronic illnesses, or rehabilitating from injuries. Your expertise will be vital in helping patients regain strength, mobility, and the ability to perform everyday activities, ultimately enhancing their quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• At least one (1) year of experience as a licensed physical therapist, preferably in an acute care or rehabilitation setting.
• A current Physical Therapist (PT) license in good standing to practice in Florida.
• Current CPR certification to ensure safety and preparedness in emergency situations.
• Valid driver's license and automobile insurance for potential outreach or home assessments.
• Strong interpersonal skills and a commitment to providing excellent patient care.
• Ability to make informed decisions and take responsibility for the outcomes of those decisions.

Preferred Experience

• Experience in a rehabilitation facility or similar healthcare setting.
