Supremacy Body & Pain Center is on the lookout for a dedicated and experienced Physical Therapist (PT) to join our dynamic team in Jacksonville, Florida. Key Responsibilities

• Conduct thorough evaluations of patients, taking into account their history and performing specific assessments to determine their unique needs and develop individualized treatment plans.
• Implement treatment protocols grounded in the latest evidence-based practices, regularly updating the approach based on patient progress and feedback.
• Maintain consistent documentation of care activities, including initial evaluations, treatment plans, progress notes, and discharge summaries, ensuring clarity and compliance with all relevant regulations.
• Consult with multidisciplinary teams to optimize patient care, making modifications to treatment plans as necessary to address any changes in patient conditions.
• Engage actively in team meetings and participate in initiatives aimed at improving access to rehabilitation services and enhancing overall patient experience.
• Ensure compliance with corporate policies and state regulations, completing all necessary training and certifications to maintain licensure and professional standards.

Patient Population / Client Focus

In this role, you will primarily work with outpatient orthopedic patients, including those recovering from surgeries, injuries, and chronic conditions. You will evaluate and treat individuals of various ages, each with distinct needs and goals. By fostering relationships built on trust and commitment, you will play a vital role in empowering patients to regain their independence and improve their quality of life.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Active state license as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.
• A minimum of two years of experience in an outpatient orthopedic setting.
• Current CPR certification.

Preferred Experience

• Experience with specialized manual therapy techniques and exercise prescription.
• Familiarity with electronic health records (EHR) systems for efficient documentation and care management.
• Previous involvement in patient education and community outreach programs to promote health and wellness.

Compensation and Benefits

This position offers an annual salary range of USD 100,000 to 120,000, commensurate with experience and qualifications, alongside a comprehensive benefits package that includes:
