Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating emotional, mental, social, and environmental factors to inform care strategies.

• Develop personalized care plans in collaboration with clients and their families, setting specific, measurable goals for achieving optimal outcomes.

• Provide individual and family counseling sessions, guiding clients through significant life transitions such as bereavement, health decline, and relocation.

• Facilitate access to community-based resources and services, advocating for clients’ needs and ensuring they receive adequate support.

• Maintain accurate client records, documenting progress and treatment plans in compliance with both ethical standards and state regulations.

• Engage in multidisciplinary team meetings to discuss client progress and collaborate on care strategies.

Patient Population / Client Focus\n

This position primarily involves working with senior citizens, many of whom may experience complex emotional and social challenges related to aging, chronic health issues, and life transitions. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• A minimum of two years of experience in social work, preferably within a community health environment or focusing on geriatric populations.

Compensation and Benefits\n
