Key Responsibilities

• Conduct in-depth physical therapy evaluations to assess residents' mobility, strength, range of motion, and functional abilities, establishing baseline measures for treatment planning.
• Create and implement personalized treatment plans that target vital functional goals, emphasizing improvements in mobility, balance, strength, and daily living activities.
• Utilize therapeutic exercises, manual therapy techniques, and assistive devices tailored to individual patient needs to enhance rehabilitation outcomes.
• Implement pain management strategies, incorporating therapeutic modalities as appropriate, while supporting residents in fall prevention and safety awareness.
• Track patient progress meticulously, documenting treatment sessions, outcomes, and adjusting therapy approaches based on evolving patient needs and clinical guidelines.
• Educate patients and their families about rehabilitation exercises, postural awareness, and best practices for continuing improvement beyond the rehabilitation period.
• Collaborate with interdisciplinary team members, including nurses and physicians, to ensure comprehensive patient care, aligning therapy goals with overall health management plans.
• Maintain accurate clinical documentation that complies with healthcare regulations and standards.

Qualifications

• Must possess an active and valid Physical Therapy license in Virginia.

Compensation and Benefits
