Full-Time Licensed Physical Therapist for Home Care Services

Position Overview

We are seeking a dedicated and passionate Physical Therapist to join our team at the Center for Physical and Rehabilitation, LLC in Woodbridge, Virginia. This full-time position focuses on providing high-quality rehabilitation services through home care, allowing patients to improve their mobility and quality of life in the comfort of their own environment. The ideal candidate will possess at least two years of experience in the field and hold a relevant Bachelor's degree.

Work Environment

This is an onsite position focused on home care, allowing therapists to interact with patients in their own settings. Working hours are from 9 AM to 5 PM, Monday through Friday. By working directly in patients' homes, you will have the opportunity to observe their daily challenges and successes, tailoring your approach to best suit their individual conditions.

Key Responsibilities

• Conduct thorough evaluations of patients to develop comprehensive, personalized treatment plans aimed at restoring function and mobility.
• Provide one-on-one therapy sessions that may include manual therapy, therapeutic exercises, and modalities to enhance recovery.
• Educate patients and their families about exercises, techniques, and strategies to continue rehabilitation effectively outside of therapy sessions.
• Monitor patient progress, modifying treatment plans as necessary to ensure optimal outcomes throughout the therapy process.
• Develop and implement innovative exercise programs targeted at improving strength, flexibility, balance, and overall fitness.
• Utilize evidence-based practices and stay current with the latest physical therapy trends and research for continuous improvement.
• Maintain accurate, detailed records of treatment provided and patient progress using electronic medical records (EMR) software, specifically WebPT.
• Collaborate with a multidisciplinary healthcare team to ensure integrated patient care and optimal treatment outcomes.

Patient Population / Client Focus

Our patient population includes individuals of all ages requiring rehabilitation due to injuries, surgeries, chronic conditions, and other medical diagnoses. As a Physical Therapist in home care settings, you will work closely with patients who may have mobility challenges and require tailored interventions to safely regain their independence. Through your dedicated efforts, you will greatly contribute to enhancing your patients' lifestyle and recovery.

Qualifications

• A Bachelor's degree in Physical Therapy or equivalent from an accredited institution.
• An active Physical Therapist license in the state of Virginia with a clean professional record free of any sanctions or board proceedings.
• Strong proficiency in various manual therapy techniques and an adeptness for developing successful exercise regimens.
• Experience with electronic medical records (EMR) systems, particularly WebPT, is preferred.
• Up-to-date knowledge of contemporary physical therapy practices and rehabilitation techniques.

Preferred Experience

• A minimum of two years of experience in physical therapy practice, particularly in outpatient or home care settings.
• Experience working with diverse patient populations, managing varying case complexities and treatment plans.

Compensation and Benefits
