We are seeking an experienced Physical Therapist (PT) Home Health for a 13-week contract serving Walnut Creek and surrounding Contra Costa County communities in California. The Physical Therapist will provide skilled home-based rehabilitation services, completing patient evaluations, treatment visits, reassessments, and required OASIS-related documentation.

The assignment requires regular travel throughout the designated service territory, with occasional travel outside the primary area and weekend coverage as needed to maintain patient visit expectations. The expected productivity standard is at least 27 points per week for guaranteed-hour eligibility.

• Evaluate home health patients and develop individualized physical therapy treatment plans based on functional limitations and rehabilitation goals.

• Provide skilled physical therapy interventions designed to improve mobility, strength, balance, safety, and functional independence in the home environment.

• Complete evaluations, reassessments, routine visits, discharge visits, and applicable OASIS documentation according to home health requirements.

• Educate patients and caregivers regarding therapeutic exercises, mobility techniques, home safety, and strategies to support continued functional improvement.

• Travel throughout the assigned service territory and outside the primary area when necessary while maintaining required weekly productivity and patient visit expectations.

• Minimum 1 Year of Physical Therapist Experience Required; Home Health Experience Strongly Preferred.

• Current, Active California Physical Therapist License Required.

• Current BLS Certification Required.

• Valid Driver's License, Current Auto Insurance, and Vehicle Registration Required for Home Health Travel.

• Ability to Meet a Minimum Productivity Expectation of 27 Points Per Week, Travel Throughout the Assigned Territory, Work Weekends When Required, and Complete All Required Health and Credentialing Requirements.
