Key Responsibilities\n

As a Mental Health Therapist at Gracious Growth Counseling Center, you will be responsible for a variety of tasks, including but not limited to:

• Conducting thorough psychosocial assessments for individuals, adolescents, families, and groups to determine their mental health needs and formulate treatment plans accordingly.

• Providing graduate-level clinical social work and counseling interventions tailored to client-specific needs.

• Completing initial comprehensive patient and family psychological evaluations, and triaging care based on the assessment outcomes.

• Collaborating with multidisciplinary teams, ensuring effective communication regarding treatment plans via documentation and regular team meetings.

• Delivering compassionate case management for assigned patients, which includes consultation and collaboration with external healthcare professionals and community resources to promote comprehensive care.

• Maintaining high standards of ethical documentation, adhering to departmental expectations.
\n
• Implementing Cognitive Behavioral Therapy (CBT) and other evidence-based treatment modalities to facilitate the completion of individualized service plans for clients.
